This legal document provides you with the opportunity to appoint a person/s to act on your behalf in relation to future medical care, treatment and welfare, if you are unable to, or become incapacitated.

Your Advanced Care Directives should be periodically reviewed, especially if your circumstances change such as having children, marriage, divorce, death of beneficiaries or a change in your financial situation.
